2017 Emerging Leader Application

Thank you for applying for the 2017 AWHONN Emerging Leader Program. In order for your application to be considered complete, you will need to fill out the form below, include one reference, and have identified a mentor and submitted the associated paperwork. Download the Reference Form and Mentor Form.

 

Please fill out all pertinent fields. Required fields are marked with an asterisk. You will also be required to attach your resume/CV at the end of the application. Once you submit your form, be sure to that you pass along the Reference Form to a professional colleague. Have them fill it out and submit it no later than October 29. Additionally, there is a document pertaining to your mentor. Please identify a mentor and have them sign this form and return.

 

Note: your mentor and your reference CAN NOT be the same person. Instructions for submitting the references and mentor sheet are on the forms. All application materials are due no later than October 29, 2016. Applicants chosen for the program will be required to sign conflict of interest documents as well as an ethical governance statement.

 

 

List any special awards/honors/recognitions that you feel may be relevant in the selection process and demonstrative of your leadership potential. *
Briefly describe what a career in nursing has meant to you and why you would describe yourself as a current or potential nurse leader
Briefly describe your view of AWHONN, any previous volunteer involvement with the association and why you are seeking this program opportunity.
List any community involvement/volunteer activities that you may deem relevant to your application, especially those where you have held office or served on Boards/Committees.
Briefly describe your career/leadership goals: (for example, become an advanced practice nurse, acquire my PhD degree, become a nurse manager/health care executive, serve as President of my professional society, etc.)
Briefly describe the kinds of leadership activities in which you hope to engage as an AWHONN emerging leader: (for example, serve on an AWHONN national ad hoc body, serve as a section or chapter leader, mentor new nursing grads, lead a local Journal Club, become a journal author/editor/reviewer, recruit new AWHONN members, etc.)
Briefly describe a leader(s) that you most admire and why?
What motivates you routinely to step up and lead?
What are your expectations of this program? What do you hope to get out of it?
How do you envision sharing your experiences as an AWHONN Emerging Leader with other nurses (both members and non-members)?
Does your supervisor/employer support your application and participation in the Emerging Leader Program? Are they aware that you will need to take 3 days off in Feb/Mar, a week in June and 3 days in October/November for mandatory activities in DC and LA?
