2019 AWHONN Emerging Leader Application

Thank you for applying for the 2019 AWHONN Emerging Leader Program. AWHONN’s Emerging Leader Program seeks to build upon the leadership skills and acumen candidates have already established within their organizations, and who seek opportunities to grow within AWHONN. This program also heavily relies on the use of mentors. Each candidate must have secured a mentor that will provide support and guidance during the year-long program. Mentor roles and responsibilities are included with the mentor agreement form required for a complete application. During the course of the program, each participant will be expected to complete a project that will positively impact a Chapter, Section or National Committee, and that is measurable in its success.

In order for your application to be considered complete, you will need to fill out this application, provide one (1) professional reference, and have identified a mentor and submitted the associated paperwork. Please note that your reference and mentor CANNOT be the same individual. Links to reference forms and mentor agreement  forms are located below. Instructions for submitting the references and mentor sheet are on the forms.

Please fill out all pertinent fields. Required fields are marked with a red asterisk. You will also be required to attach your CV and cover letter at the end of the application.

All application materials are due no later than November 15, 2018. 

Applicants chosen for the program will be required to sign conflict of interest documents as well as an ethical governance statement. 

There will be a $375 fee for those agreeing to participate in the 2019 AWHONN Emerging Leaders program.
